1 . Barangay Clearance and Cedula (1 original copy)

2 . OR and CR for the Motorela (1 original copy)

3 . Insurance Policy (1 original copy)

4 . Certification from the Association President (1 original copy)

5 . Sanitary Permit for Rig (1 original copy)

6 . Waiver (Second-hand Unit)

Procedures

1 . Submit complete required requirements

2 . Proceed to City Treasurer’s Office for assessment and payment.

3 . Client return to Permits and License Division and present the Official Receipt

4 . Client receives the printed Special Permit, proceed to City Mayor’s Office and have it approved by the City Mayor
