3 . Prosecutor’s Clearance (1 original copy)

4 . MTCC Clearance (1 original copy)

5 . RTC Clearance (1 original copy)

Procedures

1 . Submit all the requirements for verification and completeness

2 . Log in the client’s logbook

3 . Receive the document with city seal to be signed by the City Mayor at the CMO Annex Office
