1 . Birth Certificate of Minor Child/ren (xerox /original)

2 . Barangay Certificate of Residency (1 copy)

3 . Solo Parent Certification from the Barangay Captain

4 . 1x1 ID Picture (2 pcs.)

5 . School Certification(1 copy)

6 . Death Certificate ( 1 copy if applicable)

7 . Medical Certificate for Special Child/ren and PWD (1 copy)

8 . Court Decision ( 1 copy if necessary)

9 . Latest Income Tax Return( 1 copy If applicable)

Procedures

1 . Proceed to the office and submit the requirements

2 . Affix signature in the Log book and receive the Solo Parent I. D.
