1 . OSCA ID:1 copy original, 1 photocopy

2 . Training Certificate if any, 1 copy original, 1 photocopy

3 . Barangay Certificate of Good Moral Character; 1 copy original, 1 photocopy

4 . Certification from the group association President (if member of an organization) original

Procedures

1 . Proceed to the CSWD Senior Citizen Welfare Division and submit requirements

2 . Receive the Referral Letter and Certification, proceed to agency being referred
