Requirements

1 . Birth Certificate (1 original copy)

2 . Birth Certificate of the eldest and the 2nd child of the centenarian (1 original copy)

3 . If late registration of birth certificate submit the following:

 - Birth certificates of the 2 disinterested persons not late registered and aged 80 years old and above (1 original copy) 

 - Joint affidavit of 2 disinterested persons (1 original copy) 

4 . Barangay. Certificate of Residency (1 original copy)

5 . OSCA ID (3 Photocopies)
