• Property Index Number (PIN#)
• Sanitary Permit
• Fire Safety Inspection Certificate
• CEMO Certification
• EEDMO Awards & Certification (Owned by Govt)
• Other permits/clearances relative to the line of business applied

For Partnership, Corporation and Cooperative:(original copies)

• SEC Registration and Articles of Incorporation
• CDA Certificate for Cooperative
• Brgy. Clearance
• Lease Contract/ Proof of ownership (Notarized)
• Occupancy Permit/ Blg. Permit
• Compliance of Zoning Ordinance (if applicable)
• Property Index Number (PIN#)
• Sanitary Permit
• Fire Safety Inspection Certificate
• CEMO Certification
• Other permits/clearances relative to the line of business applied

2 . For Renewal of Business Permit:

• Basis for computing taxes, fees and charges [Income Tax Return]
• Sanitary Permit
• Occupancy Permit/Bldg. Permit
• Lease Contract/Proof of Ownership (Notarized)
• Fire Safety Inspection Certificate
• CEMO Certification

1 . Secure Application for Business Permit form and checklist of requirements as to type of business applied for

2 . Submit all the documents, together with Official Receipts.

3 . Receive the Approved Business Permit and Business Plate.
