2 . Hospital Bill

3 . Diagnostic Request with indicated amount/fee

Procedures

1 . For Non-PhilHealth Member Patients (opt to pay); Approach the Billing Clerk and ask for Statement of Account (SOA)

2 . Wait for the SOA to be released

3 . Present Hospital Bill/Diagnostic request with corresponding fee

4 . Receive Official Receipt (OR)
