Requirements
1 . Referral Form and Trip Ticket
Procedures
1 . If transfer to Hospital of Choice (HOC), client seeks Doctor’s approval
2 . Client will wait while the referral is facilitated.
3 . Significant Others (SO) will process their hospital billing charges
4 . Photocopy pertinent documents (e.g. official diagnostic examination results)
5 . Present settled hospital bill to the Unit Nurse on Duty, and secure discharge slip.
6 . Client prepares for transport
7 . Client & SO’s respond to EOD’s queries
8 . Clients & SO’s (max of 2) coordinates with HEMB Emergency Officer on Duty
9 . Significant Others stays with client/patient on bedside & provide necessary patient’s information as needed by receiving facility
