2 . Birth Certificate of the Deceased

3 . Marriage Certificate of the Deceased

4 . Government Issued Identification Card

5 . Informant/Next of KIN/Husband,Wife, Mother, Father, Children with I.D

6 . Other Informant Authorization with Valid I.D and affidavit

7 . Certificate of Residency

8 . Certificate of Occurrence of Death

9 . Certificate of Indigency

10 . Endorsement to City Prosecutor

11 . Posting Period(10 days)

12 . Police Blotter

13 . SOCO Report

14 . Attachments: Muslim Filipino/Indigenous People

15 . IMAM/Indigenous I.D

16 . Official Receipt

Procedures

1 . Sign in the Client Log Book in the office

2 . Submit the checklist to Desk 2 for initial verification and assessment of documents

3 . Return to City Civil Registrar Office to submit the required documents

4 . Proceed to City Health Office and Embalmer

5 . Return to the City Civil Registrar’s Office

6 . Pay the required fees at the City Treasurer’s Office by showing the Order of Payment.

7 . Return to the City Civil Registrar’s Office

8 . Proceed to City Prosecutor’s Office

9 . Return to the City Civil Registrar’s Office

10 . Receive the Certificate of Death
