3 . Government Issued Identification Card
4 . Informant/Next of KIN/Husband,Wife, Mother, Father, Children with I.D
5 . Other Informant Authorization with Valid I.D and affidavit
6 . Certificate of Residency
7 . Certificate in Occurrence of Death
8 . Certificate of Indigency
11 . Attachments: Muslim Filipino/Indigenous People
Procedures
1 . Sign in the Client Log Book in the office
2 . Submit the checklist to Desk 2 for initial verification and assessment of documents
3 . Proceed to City Health Office and Embalmer
4 . Return the documents to the City Civil Registrar’s Office
5 . Receive the Certificate of Death
