1 . Four (4) Copies of Marriage Certificate

2 . Certificate of No Marriage (CENOMAR)

3 . Death Certificate for widow/er

4 . Permit to hold ceremony outside the church or court

5 . Attachments: Muslim Filipino/Indigenous People

6 . Affidavit of two witnesses

7 . Affidavit of Cohabitation

8 . Government Issued Identification Card

9 . Endorsement to City Prosecutor

10 . Posting Period(10 days)

11 . Official Receipt

12 . CRS Form No.1(Negative Certification of Marriage)

Procedures

1 . Sign in the Client Log Book in the office

2 . Submit the checklist to Desk 3 for assessment

3 . Pay the required fees at the City Treasurer’s Office by showing order of payment

4 . Return to the City Civil Registrar’s Office with the Official Receipt

5 . Proceed to the City Prosecutor’s Office to submit documents

6 . Return to the City Civil Registrar’s Office

7 . Receive the Certificate of Marriage
