3 . Death Certificate for widow/er
4 . Permit to hold ceremony outside Church / Court
5 . Attachments: Muslim Filipino/Indigenous People
6 . Affidavit of Cohabitation
7 . Government Issued Identification Card
Procedures
1 . Sign in the Client Log Book in the office
2 . Submit the checklist to Desk 4 for initial assessment
3 . Submit the Marriage Certificate and Attachments
4 . Receive the Marriage Certificate
