1. CRS Form No.1(Negative Certification of Birth)
2. Affidavit of two witnesses
3. Affidavit of corroboration
4. Official Receipt
5. Postal Money Order
6. Government Issued Identification Card
7. Attachments for Muslim Filipino/Indigenous People
8. Attending Traditional Midwife/Traditional Hilot

2 . Additional Requirement for Legitimate

1. Marriage Certificate of Parents

2. Certification from IMAM or Tribal Leaders

3. Remittances

4. 3R Picture of Child

3 . At least two (2) of the following:

• Baptismal Certificate
• Philhealth MDR
• Police Clearance
• NBI Clearance
• Income Tax Return
• Medical Record book of the child
• Immunization Record of the child from 0-5 years old
• Form 137 / Transcript of Record
• Transcript of Records
• Service Record
• CEDULA
• OFW Record
• Marriage certificate of applicant

4 . Additional Requirement for Illegitimate

1. AUSF (Affidavit to Use the Surname of the
Father)
2. Affidavit of Guardianship/Abandonment

Procedures

1 . Sign in the Client Log Book in the office front desk

2 . Submit the checklist to Desk 1 for initial assessment and verification.

3 . Return to City Civil Registrar’s Office to submit the required documents

4 . Pay the required fees at the City Treasurer’s Office by showing the Order of Payment.

5 . Return to the City Civil Registrar’s Office with the Official Receipt

6 . Receive and mail the documents
