Requirements

1 . For Legitimate and Illegitimate

 1. CRS Form No.1(Negative Certification of Birth) 

 2. Affidavit of two witnesses 

 3. Official Receipt 

 4. Posting Period(10 days) 

 5. Government Issued Identification Card 

 6. Attachments: Muslim Filipino/Indigenous People 

 7. Endorsement to City Prosecutor 

 8. Or any two (2) of the following: 

 a. Baptismal Certificate 

 b. Philhealth MDR 

 c. Police Clearance 

 d. NBI Clearance 

 e. Income Tax Return 

 f. Medical Record book of the child 

 g. Immunization Record of the child from 0-5 years old 

 h. Form 137 / Transcript of Record 

 i. Transcript of Records 

 j. Service Record 

 k. CEDULA 

 l. OFW Record 

 m. Marriage certificate of applicant 


2 . Additional Requirement for Legitimate

 1. Marriage Certificate Parents

 2. Certification from IMAM or Tribal Leaders (for Muslim or Indigenous group) 

 

3 . Additional Requirement for Illegitimate

 1. Affidavit of Guardianship/Abandonment 

 2. AUSF (Affidavit to Use the Surname of the Father) 

Procedures

1 . Sign in the Client Log Book in the office front desk

2 . Submit the checklist to Desk 1 for initial assessment and verification.

3 . Return to City Civil Registrar’s Office to submit the required documents

4 . Pay the required fees at the City Treasurer’s Office by showing the Order of Payment.

5 . Return to the City Civil Registrar’s Office with the Official Receipt

6 . Proceed to the City Prosecutor’s Office

7 . Return to the City Civil Registrar’s Office

8 . Receive the Certificate of Live Birth
