1 . Latest Barangay Indigency(1 Original copy)

2 . Birth Certificate of the child(1 Original copy)

3 . Marriage Contract of Parents(1 Original copy)

4 . Recent Report Card (1 Original copy) for presentation and cross-checking (1 Original copy) for submission

5 . School Certification stating that the grantee is enrolled for the current S.Y. and is not under/covered of any other scholarship granrts

6 . Certification that the child is not 4Ps & 5Ps beneficiary

7 . 1 x 1 ID picture of student/pupil

8 . 1 x 1 ID Parents

Procedures

1 . Applicant parents/ guardians present one’s self for interview or validation regarding their SAGIPS application form data

2 . Presentation and submission of requirements of the qualified SAGIPS scholar
