1 . Duly accomplished and subscribed Sworn Statement with all supporting documents (w/ CASSO control No.)

2 . Sketch Plan/ Vicinity Map of location of the Property signed by Owner

Procedures

1 . Submits complete documents in accordance with the listed requirements

2 . Receives document

3 . Be at the area during inspection (upon receipt of call or message on the schedule of inspection)

4 . Conform data in RPA Form No.1- C (FAAS) through signing

5 . Listens and note instructions

6 . Claim Tax Declaration and present the requirement/s needed for releasing

7 . Receive approved Tax Declaration
