1 . Duly accomplished Request for Correction/ Change of Entry Form

2 . Latest Tax Receipt or Tax Clearance Certificate

3 . Electronic Copy of Title or any valid documents to support the request

4 . Valid documents

 - Valid ID 

- Photocopy of Title (Bring Original for crosschecking) 

-Other supporting documents (i.e, Birth Certificate, Approved Plan, Etc.) 

Procedures

1 . Submits complete documents in accordance with the listed requirements

3 . Listen to the instruction in securing Certified True Copy of Tax Declaration
