1 . Duly accomplished Request for Correction/ Change of Entry Form
2 . Notarized Affidavit for correction of clerical or typographical error a misspelled name, family name, middle name, etc. indicated in the tax declaration.
3 . Electronic Copy of Title or any valid documents to support the request for correction of lot area
4 . Latest Tax Receipt or Tax Clearance Certificate
5 . Photocopy of title with annotation of Registry of Deeds entry number regarding the corrections (bring original for crosschecking)
Procedures
1 . Submit complete documents in accordance with the listed requirements
2 . If with lacking requirements, receive documents
3 . If documents submitted is complete, wait for call/text for the schedule of inspection
