3 . Certificate of Business Operation Retirement (if engaged in Business)

4 . Demolition Permit

5 . Certification for Demolished Building

6 . Sketch Plan/ Vicinity Map of the property location, signed by the owner

Procedures

1 . Submit complete documents in accordance with the listed requirements

 Be at the area during inspection 

2 . If documents submitted is complete, wait for call/text for the schedule of inspection

3 . Be at the area during inspection

4 . Conform data in Field Assessment and Appraisal Sheet (FAAS) through signing

5 . Note instructions. Follow-up through phone call on agreed schedule

6 . Present requirement/s for releasing
