Requirements

1 . Building Permit with Building Plan and Certificate of Occupancy

2 . Consent and Authority of the Lot Owner(received by Building Owner/Administrator)

3 . Affidavit of Undertaking for government lots

4 . Accomplished affidavit of ownership or Sworn Statement of the market value of the property, in the absence of a building permit or Certificates required under Item 1.

5 . Sketch Plan/ Vicinity Map of the Property signed by Owner

Procedures

1 . Submits complete documents in accordance with the listed requirements

2 . Receives documents, if requirement is lacking

3 . Note/write down instructions

4 . Be at the area during inspection (upon receipt of the call or message on the schedule of inspection)

5 . Conform data in RPA Form No.1-A (FAAS) by affixing signatures

6 . Notes instructions and follow-up through phone call on agreed schedule

7 . Present requirement/s for releasing

8 . Receives approved Tax Declaration
