1 . Official receipt for Service Record

2 . If the request is filed by a representative:

 • Authorization Letter and/or 

• Special Power of Attorney (SPA); and 

• One (1) valid ID of the representative

2 . Receive Certificate of Employment and Sign the Releasing Logbook
