2 . Notarized Personal Data Sheet (CSC Form No. 212, Revised 2017) with recent passportsized picture)

3 . Work Experience Sheet (if applicable)

4 . Latest Individual Performance Commitment and Review Rating or Performance Rating/Appraisal/Certification (if applicable)

5 . Photocopy of training and seminar certificate

6 . Photocopy of certificate of eligibility/rating/valid PRC license

7 . Photocopy of Transcript of Records
