1 . Letter request

2 . One (1) valid identification card (ID)

3 . If the request is filed by a representative

 • Authorization Letter and/or 

• Special Power of Attorney (SPA); and 

• One (1) valid ID of the representative 

Procedures

1 . Submit all required documents

2 . Wait while the requested documents/ records are being retrieved

3 . Receive the documents/ records requested
