Requirements

1 . Donors are physically fit (18 to 60 years of age with normal blood pressure, free from any sickness, normal weight, etc.)

2 . 1 Authenticated copy of NSO birth certificate

3 . Signed Waiver from the parents (Donors below 18 years old)

Procedures

1 . Prospect donors approaches the person incharge

2 . Fills up waiver form by new donor for bloodletting

3 . Fill up the Donor’s Registration Form

4 . Client proceeds to the specific venue

5 . Take advice from staff

6 . Receive Blood Donor Card
