1 . For New Business: Single Proprietorship

  • DTI Registration
  • Brgy. Clearance
  • Occupancy Permit/Bldg. Permit
  • Lease Contract/Proof of ownership
  • Compliance of Zoning Ordinance (if applicable)
  • Property Index Number (PIN#)
  • Sanitary Permit
  • Fire Safety Inspection Certificate 
  • CEMO Certification
  • EEDMO Awards & Certification (Owned by Gov't)
  • Other permits/clearances relative to the line of business applied

2 . For New Business: Partnership, Corporation and Cooperative

(original copies)

  • SEC Registration and Articles of Incorporation
  • CDA Certificate for Cooperative
  • Brgy. Clearance
  • Lease Contract/Proof of ownership (Notarized)
  • Occupancy Permit/Bldg. Permit
  • Compliance of Zoning Ordinance (if applicable)
  • Property Index Number (PIN#)
  • Sanitary Permit
  • Fire Safety Inspection Certificate
  • CEMO Application
  • Other permits/clearances relative to the line of business applied

3 . Renewal of Business Permit

(original copies)

  • Basis for computing taxes, fees and charges (Income Tax Return)
  • Sanitary Permit
  • Occupancy Permit/Bldg. Permit
  • Lease Contract/Proof of Ownership (Notarized)
  • Fire Safety Inspection Certificate
  • CEMO Certification
  • EEDMO Award & Certification (Owned by Gov't)
  • Philhealth/SSS/Pag-ibig - (Based on No. of Employees)

Procedures

1 . Secure Application for Business Permit form and checklist of requirements as of type of business applied for.

2 . Submit all the documents, together with Official Receipts.

3 . Receive the Approved Business Permit and Business Plate.
